COVID-19 Vaccines 

GENERAL

Who is eligible to receive a vaccine?

As of Monday, April 5, 2021 all Florida residents shall be eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration.

PERSONS AGES 12-17: The Pfizer vaccine is authorized for persons age 12 and up. The Moderna and Janssen (Johnson and Johnson) vaccines are authorized for persons age 18 and up. All individuals under the age of 18 receiving a vaccine must be accompanied by a guardian and complete the COVID-19 vaccine screening and consent form. 

PERSONS AGES 5-11: On Tuesday, November 2, 2021, the Centers for Disease Control and Prevention (CDC) Director, Rochelle P. Walensky, M.D., M.P.H., endorsed the CDC Advisory Committee on Immunization Practices’ (ACIP) recommendation that children ages 5-11 be vaccinated against COVID-19 with the Pfizer-BioNTech COVID-19 pediatric vaccine.

CDC has expanded vaccine recommendations for the use of Pfizer-BioNTech COVID-19 vaccination to these children, ages 5-11, along with the continued recommendation for individuals 12 years and older in the US.

 

APPOINTMENTS

How do I make an appointment for a vaccine?

DOH-Bay is administering the Moderna vaccine at our clinic located at 597 W 11th Street, Panama City on Tuesdays by appointment. We are also administering 3rd dose and booster doses of the Moderna vaccine to individuals eligible per CDC guidelines. Please call (850) 872-4455, select option 1, to schedule an appointment. 

Moderna and Pfizer 3rd Dose Eligibility:

Individuals at least 18 years of age who have undergone solid organ transplantation, or who are diagnosed with conditions that are considered to have an equivalent level of immunocompromise and at least 28 days after completion of 2nd dose.

Pfizer-BioNTech and Moderna COVID-19 Booster Vaccines Eligibility:

Individuals at least 18 years of age and older who have completed their initial COVID-19 vaccination series, at least 5 months have passed since the completion of their second dose.

Johnson & Johnson Booster Vaccine:

The Johnson & Johnson booster is recommended 2 months after the 1st dose and it is recommended that a MRNA (Moderna or Pfizer) be given as the booster.

How do I replace my COVID-19 vaccination card?

DOH-Bay does not issue replacement COVID-19 vaccination cards. Instead, individuals who were vaccinated in Florida can get a copy of their Florida shot record. Please follow one of the steps below to get a copy of your shot record:

  • Submit your request directly to Florida SHOTS: You can request your COVID-19 vaccination records directly from Florida SHOTS by filling out the Florida Department of Health form – DH3203 Authorization to Disclose Confidential Information form online, electronically sign and submit it here.  
  • Email DOH-Bay at baymedicalrecords@flhealth.gov. Put Immunizations in the subject line. Instructions and a consent form will be emailed back to you.
  • Send a fax to (850) 747-5475 Attn: Immunizations. You will receive instructions on how to obtain your Florida shot record.
  • Come to our office at 597 W 11th Street, Panama City and enter through the Clinic entrance. Sign in at the kiosk and someone will assist you.
  • Call (850) 872-4455, select option 1 to make an appointment.

Copies of your Florida shot record are free of charge.

Do I need to be a resident of Florida to receive the COVID-19 vaccine?
State Surgeon General Scott Rivkees issued a Public Health Advisory on April 29 authorizing a COVID-19 vaccine to any eligible individual (as prescribed by the Food and Drug Administration) who is a Florida resident or who is present in Florida for the purpose of providing goods or services for the benefit of residents and visitors of the State of Florida.

To prove residency an adult resident must provide a copy of his or her valid Florida driver license or a copy of a valid Florida identification card.

Seasonal residents may provide a copy of two of the following to show proof of residential address:

  1. A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
  2. One proof of residential address from the season resident’s parent, step-parent or legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
  3. A utility hookup or work order dated within 60 days before registration.
  4. A utility bill, not more than 2 months old.
  5. Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
  6. Mail from a federal, state, county, or municipal government agency, not more than 2 months old.

Seasonal resident means any person who temporarily resides in this state for a period of at least 31 consecutive days in each calendar year, maintains a temporary residence in this state, returns to the state or jurisdiction of his or her residence at least one time during each calendar year, and is registered to vote or pays income tax in another state or jurisdiction.

COVID-19 testing at DOH-Bay is temporarily suspended to prioritize vaccines. 

Other COVID-19 Testing Options
There are numerous community partners and private practitioners providing COVID-19 testing in Bay County. Some testing sites are free, and some require a fee. Individuals should refrain from going to local emergency rooms for testing.
