DOH-BAY PROMOTES THE 2022 COMMUNITY HEALTH ASSESSMENT SURVEY

May 06, 2022

Panama City, Fla. — The Florida Department of Health in Bay County, in partnership with the Bay County Community Health Assessment (CHA) steering committee, invites you to participate in a research study for community health needs. The survey asks questions about the health of the community and once the data is collected, the health needs are prioritized to focus on the communities’ greatest needs. This information can help develop a community health improvement plan by justifying how and where resources should be allocated.

“You may ask yourself, why do we need a health assessment survey? Your participation in the survey will help identify gaps in resources, services and our community’s health and nutrition needs. Housing, schools, workplaces, transportation, parks & recreation, and healthcare are an integral part of our habitat. Therefore, closing gaps in our built environment and in public health will contribute to creating a healthier community,” said Sandon S. Speedling, MSH, CPM, CPH, Florida Department of Health in Bay County, Administrator and Health Officer.

We want to know how you view our community; your opinions are important. The survey link will go live on May 9 and end on May 31. The questionnaire will take approximately 5-10 minutes to complete, and all responses are confidential. Please visit: https://www.surveymonkey.com/r/SKCSW3H  or scan the QR Code below:
