Our Immunization Program works to prevent the spread of vaccine-preventable diseases in Bay County. We offer free immunizations for uninsured and underinsured children ages 1 month through 18 years of age.
Underinsured includes those in which insurance does not pay for immunizations. We also offer adult immunizations for a fee.
Clinic Hours
Monday, Tuesday, Thursday, and Friday: 7:30 a.m. – 4:30 p.m.
Wednesday: 10 a.m. – 4:30 p.m.
All immunization services are by appointment only.
To schedule an immunizations appointments, call 850-872-4455 and press 1 for appointments, then 1 for medical appointments.

- Out of state records transferred to Florida DH 680 Form (for school registration), $20 fee
- DH 680 Form, $10 fee
- College Forms (for college entry), $10 fee
- Religious exemption
- Immigration immunizations
- International travel immunizations - view traveler's health for reccomendations on vaccines
For record transfers, DH 680 Form and College Forms, please bring the following:
- Copy of Immunization records (must be translated to English)
- Parent picture ID/passport
- If needed, a copy of guardianship paperwork (from Florida Department of Children and Families, court documents, or power of attorney)
- Insurance card (if applicable)
- Optional: birth certificate and SSN card
Fees can be paid by cash, check, money order, Visa, or Mastercard.
