The primary goal of our Immunization program is to prevent the spread of vaccine-preventable diseases in Bay County. We offer free immunizations for uninsured and underinsured children ages 1 month - 18 yrs. Underinsured includes those in which insurance does not pay for immunizations. We also offer adult immunizations for a fee.
Clinic Hours
Due to our COVID-19 response, all Immunization Services will be by appointment only until further notice.
This is for our community and staff’s protection.
Please call (850) 872-4455 and wait for the COVID-19 message to end, then press 1 for Appointments then press 1 for Medical Appointments to schedule an immunizations appointment for the following services:
- Child Vaccines
- Adult Vaccines
- Religious Exemption
- Immigration Vaccines
- International Travel Vaccines
Please see a list below of services that do not require an appointment. Please email your request to BayMedicalRecords@FLHealth.gov. Please include your curremt address and a phone number. A call back will be provided when forms are ready for pick-up.

Travel immunizations are completed by appointment. Please call (850) 872-4455 to schedule an appointment.
Servicios De Immunizacion Para Estudiantes Del Distrito De Bay
Debido a la respuesta COVID-19 de DOH-Bay, todos los servicios de inmunización serán cita previa hasta nuevo aviso. Esto es para la protección de nuestra comunidad y el personal.
Llame al (850) 872-4455 y espere a que termine el mensaje COVID-19, presione 8 para español y haga su cita de servicios de inmunización; Vacunas para niños y Exención religiosa.
Consulte una lista a continuación de los servicios que no requieren una cita.
¨ Registros de fuera del estado transferidos al formulario 680 de Florida (para el registro escolar) - Envíe los registros por correo electrónico a BayMedicalRecords@FLHealth.gov con la dirección actual y el número de teléfono para devolver la llamada cuando el formulario esté listo para recogerlo.
¨ Impresión del formulario 680: llame a la línea de citas para confirmar que el formulario está listo para recogerlo. De lo contrario, deberá programar una cita para obtener una copia certificada.
