Your Visit

The Immunization Clinic is open Monday, Tuesday, Thursday, and Friday from 7:30 a.m. to 4:30 p.m. and Wednesday from 10 a.m. to 4:30 p.m. All immunization services are by appointment only. Call 850-872-4455 to schedule your appointment.

  • Please bring your picture ID and all previous immunization records for each individual needing services to your appointment
  • Prior records must be available prior to or at visit time
