The Diabetes Services Program is committed to excellence in chronic disease prevention and health promotion through education. Development of health management strategies along with education is integral to decreasing risk for development of chronic disease and managing diagnosed chronic disease.
DOH-Bay offers the following services:
National Diabetes Prevention Program
A lifestyle change program can reduce a person’s risk of developing type 2 diabetes by 58% (71% in individuals aged 65 and older). The Diabetes Prevention Program Program teaches strategies for eating healthy and incorporating physical activity into daily life through a group setting led by a lifestyle coach.
Diabetes Self-Management Education and Support
This program is for those diagnosed with diabetes to help them learn more about diabetes, have improved blood glucose levels, reduce their risk of complications, and improve their quality of life.
Advanced Diabetes Self-Management Education classes are facilitated by diabetes educators utilizing the International Diabetes Center curriculum. DOH-Bay’s program is accredited by the American Association of Diabetes Educators.
Community Education
Our staff provide presentations on topics related to chronic disease prevention and management on an as-needed basis.
Diabetes Risk Screening
Staff offer individuals the opportunity to take the diabetes risk test and have their blood screened to determine their risk for diabetes. Screenings may also be scheduled for community-wide events upon request.
Insulin Medication Assistance
The Insulin Distribution Program is a safety-net program that is available when there is no other resource available for the insulin. The Florida Department of Health’s central pharmacy and county health departments with a licensed pharmacy are authorized to fill client-specific prescriptions for insulin and diabetes-related supplies.
CHDs without a licensed pharmacy will forward prescriptions for insulin and diabetes-related supplies to the central pharmacy.
